Meeting Out of School
When you need a Plan for Meeting Outside of School form
Below is a link to the "Plan for Meeting Outside of School" form. Each meeting out of the school must have a separately signed form. Note that the parent must sign it. Here is how it works:1. If you plan to transport the student, TeamMates Office will need driver's license and insurance documentation. We keep the information on file and only needs updated when the documents expire. Here's how to provide this information:
a. Send a copy of your Driver's License (or stop by and we will copy it)
b. You can send a copy or ask your Insurance Agent to fax a copy, of the Declarations Page of your insurance policy. Note: the "card" does not work. The document must list the dollar amount of insurance you carry for bodily injury.
2. Decide on a specific activity, fill out the Plan for Meeting Outside of School form. It is fine to ask a parent to sign multiple forms for different activities/dates at one time if they are willing.
3. Give the form to your mentee to sign and to get a parent's signature.
4. Once signed by a parent, the form must come to the TeamMates Office:
a. During the school year, the school facilitator must also sign it.
b. During the summer, the parent will need to drop it off or mail it to the TeamMates Office, 5901 O Street, Lincoln, NE 68510.
5. In the summer, when it's not possible to have it signed by the school, the TeamMates Program Coordinator, Walter Powell, will be the one who must sign it in advance of the activity.
6. We provide verbal or email approval of the activity to you (and the school facilitator if school is in session).
7. You go and have fun!
Note: We're sorry if this seems complex but it is necessary for our insurance liability. You can read more about it towards the back of your training manual as well. There are some restrictions in where you go...not your own home, etc.
