Below are a number of forms that mentors may need during the year. TeamMates school facilitators all have copies of these too so you can either download them here or ask your school facilitator for a copy.- Plan for Meeting Outside of School form
If you and your mentee are going to meet outside of school, this form must be completed for each event. It requires the okay of the facilitator, parent/guardian and TeamMates Office, so be sure to start early to ensure all required signatures and permissions are obtained. - Sample Plan for Meeting Outside of School
