Thank you for your commitment to mentoring, you are making a difference in the life of a child!
Once a year we ask you to give TeamMates feedback about your match with an online survey. We hope to have 100% of the Lincoln mentors participate.
The survey has 19 questions and takes about 10 minutes. You do not have to give your name and results go directly to the State TeamMates office.
You will be asked for:
the name of the TeamMates Chapter. We are the Lincoln Chapter.
the name of the "TeamMates Coordinator". Our chapter refers to this person as a "School Facilitator". They are the person who make matches and manages the program at your school. If you need this name, refer to the list on the Lincoln TeamMates website Facilitator's List 2011-2012
